In the game magic the gathering I have a question about poison counters and attack points.?

My friend and I were playing and he introduced me to cards with poison counters. He claims the counters work as in if the monster has 5/4 attack then I get 5 poison counters. The card does not specifically say for each attack point. It just says the player gets 1/1 poison counters. I think the card should deal just one poison counter not 5. Who is right? For example one card is Rot Wolf.
